【REQUESTING YOUR CREDIT PROGRAM CERTIFICATE】

Students who are applying to graduate during the final term please finish the Credential Request Form or Certificate Request Form before May 5th for your credit program certification. After your application, we'll evaluate your requirements and award your credential as long as you've successfully completed your program requirements. You'll see your awarded credential on your transcripts on the appropriate conferral date. 

Steps to complete for your credential on the transcript:
1. Download the Credential Request Form on the website of Academic Affairs Division, Registrstion Section. (or you can just download it downbelow)
2. The Credential Request Form must be accompanied by a your academic transcript.
3. Send the forms to the 'College of Education, NPTU' to vertify --- then 'Center for Interdisciplinary Studies' --- then back to 'Academic Affairs Division, Registrstion Section'.

Steps to complete for your credit program certificate:
1. Download the Certificate Request Form on the website of Mt. Dawu College, NPTU.
2. The Certificate Request Form must be accompanied by a your academic transcript.
3. Send the forms to the 'College of Education, NPTU' to vertify --- then 'Center for Interdisciplinary Studies'.
